Qualification - Certificate Replacement Form/Statement of Results  

Please use this form to enquire about a replacement certificate or statement of results achieved. Please note that for examinations taken before 1 January 2003 we are unable to issue replacement certificate, but will issue a statement of results.

If you are requesting a replacement certificate because you sat an examination within the last three months and your details are incorrect on the certificate please email [email protected] or call + 44 (0) 1332 298101, option 3, option 1.

If you are have sat your examination within the last three months and have not received your certificate  please email [email protected] or call + 44 (0) 1332 298101, option 3, option 1.

As part of this application you will be asked to upload a copy of your photo ID (such as a passport or driving licence).

We charge a £100 fee per replacement certificate/statement to cover the costs of processing your enquiry.

We will start to process your enquiry upon receipt of payment. Certificates/statements of results will be issued within three weeks of receipt of payment.
